Clients
Please phone the clinic to make an appointment. For your initial assessment please bring any relevant referral notes or specialist reports and investigations (such as X-ray or MRI scans). If you are training and the injury is in your back or lower limbs please bring your running shoes and competing footwear (boots etc). We have shorts and gowns but you may feel more comfortable in your own ’short’ shorts and/or singlet. Please allow 5 minutes to complete our database form, outcome measure form and ACC forms (if appropriate).

Referrers
You can email me a referral or give your client a hard copy with instructions. We will send a Discharge Report at the completion of treatment and an update report after the sixth treatment if you request one.
